Every CO2 horror story starts with a candidacy or calibration mistake โ the wrong skin type, the wrong energy setting, the wrong density for that zone. We prevent this with the six-assessment screening process above, Paul Charette's specialty in skin-tone correction and pigmentation (which is exactly the complication profile CO2 can create), and a willingness to decline treatment when the risk doesn't match the benefit. Most patients plan CO2 around a specific week โ a kid's spring break, between a work trip and a wedding, or at the start of a long vacation. Days 1โ3 are home-only. Day 4โ6 you're home but peeling. Day 7 you can be in public with tinted sunscreen (school pickup, grocery run, casual outings). Day 10โ14 you're fully camera-ready. It's not invisible downtime โ it's predictable downtime, which is the version you can plan around. Level 1 compresses this to ~5 days for patients with tighter schedules.
Level 2 is the one-session decisive reset โ deeper resurfacing, 7โ10 days of real downtime, typically 1โ2 sessions for peak result. It's the right call for deep sun damage, entrenched melasma, etched lines, and moderate acne scarring. Level 1 is the lighter, staged path โ 5 days of downtime per session, typically 2โ4 sessions to reach a similar cumulative result. The choice is usually a function of (a) how much change you want, (b) how much downtime you can block out, and (c) whether you want the result in one decision or across three visits. Dermatologists are trained to treat disease โ acne, psoriasis, skin cancer. Plastic surgeons cut. Neither is specifically trained in the aesthetic artistry of CO2 resurfacing the way a pigmentation-specialist medical aesthetician is. Charette has performed hundreds of CO2 treatments and specializes in skin-tone correction โ which is the exact clinical halo you want for melasma and sun-damage resurfacing. "Medical" doesn't always mean "aesthetically calibrated." It depends on what kind of medicine.
CO2 is restoration, not transformation โ it doesn't change your face, it improves the quality of the skin on it. You will look like yourself, just with smoother texture, more even pigmentation, and visibly fewer fine lines. No one should be able to tell you've had "something done." They should just ask if you've been sleeping better.
Peak visible result is typically at 90 days post-treatment, and the result holds for years with basic maintenance: daily sunscreen, a retinoid, and one or two lighter touch-up treatments per year. CO2 is most proven and safest on Fitzpatrick IโIV. Darker skin tones require more conservative settings, careful pigmentation history review, and in some cases alternative treatments (MelanX peel, lighter fractional lasers).
